About The Position

Per-Visit Home Health Registered Nurse Vernon Hills and surrounding area! Abcor Home Health is looking for a dedicated Per-Visit Home Health Registered Nurse to join our family-owned business. Position Details: Location: Vernon Hills and surrounding area (must be within 30 minutes) Training: 2-weeks in Arlington Heights (training time depends on experience) Visits: 45-minute in-home visits, with a minimum of 5 visits per week Experience: Home Health and Nursing experience is required Key Responsibilities: Evaluate the physical, psychological, and social needs of patients during visits Develop and implement individualized care plans tailored to each patient’s condition Provide a variety of clinical services including wound care, IV management, PICC line care, G-Tube care, and Foley care Educate patients and their caregivers on medication management and chronic condition self-care Collaborate with physicians, physical therapists, and other healthcare providers to ensure comprehensive care Regularly monitor patient progress and adjust care plans as needed to achieve optimal health outcomes Qualifications: Associate’s degree in nursing (BSN preferred) Prior experience in Home Health, including familiarity with Home Health documentation Ability to work independently and as part of a team Knowledge of Home Health care guidelines, standards, and regulations Compassion, flexibility, and patience to work in diverse home environments Strong communication and interpersonal skills with a focus on patient care Why Work with Us? Requirements

  • Associate’s degree in nursing (BSN preferred)
  • Prior experience in Home Health, including familiarity with Home Health documentation
  • Ability to work independently and as part of a team
  • Knowledge of Home Health care guidelines, standards, and regulations
  • Compassion, flexibility, and patience to work in diverse home environments
  • Strong communication and interpersonal skills with a focus on patient care
  • Driver's License (Required)
