Home Health Registered Nurse – Rockford & Surrounding Area Abcor Home Health is a family-owned agency that cares for both our patients and our team. We’re looking for a compassionate Home Health Registered Nurse to join us in delivering outstanding care where it matters most—at home. If you believe in personalized care, value flexibility, and want to grow professionally in a supportive environment, this role is for you. 📍 Position Details Location: Rockford and nearby areas (within 30 minutes preferred) Training: 2 weeks in Arlington Heights (length tailored to your experience) Visits: In-home visits, ~30-45 minutes each. Minimum 5 visits per week. Experience: Home Health and nursing experience required

Requirements

An Associate’s degree in Nursing (LPN preferred)
Experience in Home Health, including OASIS documentation
Ability to work independently and as part of a team
Knowledge of Home Health guidelines and regulations
Compassion, patience, and adaptability for diverse home settings
Strong communication skills and a patient-first mindset
Valid Driver’s License (required)

Responsibilities

Assess patients' physical, emotional, and social needs during home visits
Create and follow individualized care plans
Provide clinical care: wound care, IV/PICC line management, G-Tube and Foley care
Educate patients and caregivers on medications and chronic condition management
Collaborate with doctors, therapists, and care teams
Monitor progress and adjust care plans as needed
