This role is for a casual Registered Nurse (RN) to support our Palliative care program in Durham North. It is a Visiting Nurse position, requiring a valid Ontario driver's license (G License) and access to a reliable vehicle for travel. The Palliative care program provides home nursing and end-of-life care, with nurses collaborating inter-professionally with external palliative care teams, care coordinators, physicians, nurse practitioners, and PSWs.

Requirements

Registered and in good standing with the College of Nurses of Ontario (CNO).
RN diploma or degree from a recognized college or university.
Valid First Aid or CPR Certification.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
A valid Ontario driver’s license (G License) and a reliable vehicle for traveling are required.

Nice To Haves

Membership of your Professional Association preferred (RNAO).
Acute Care and Medical-Surgical nursing experience is an asset.
Knowledge of community nursing is an asset.

Responsibilities

Provide and coordinate home health nursing services to diverse individuals, families, and groups according to professional standards and organizational policies.
Assess, monitor, and make care planning and care coordination decisions.
Evaluate care plan interventions through ongoing reassessment and evaluation results.
Ensure the quality of patient care by identifying and acting on factors affecting practice autonomy and delivery of quality care.
Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.
Support safe client care through continuous quality improvement.
Inform the nursing supervisor of client activities, concerns, or changes in condition/care plan, and submit appropriate documentation.
Maintain documentation standards according to the College of Nurses of Ontario.
Participate in a standby/on-call rotation for answering calls and responding if needed.
Participate in team meetings as required.
