About The Position

Clay County Public Health is accepting applications for one open position as a Full-Time Registered Nurse OR Full-Time Social Worker. This is an exempt position with a Monday-Friday work schedule from 8:00 AM to 4:30 PM. The hiring salary range varies depending on the specific role and qualifications (RN/BSN, RN-AD, or Social Worker). Requirements

• Bachelor’s degree in nursing or social work preferred
• Licensed in Minnesota as a registered nurse OR social work license preferred
• 1-3 years of related work experience
• Valid driver’s license
• Ability to pass a background check
• Clay County Application must be submitted for all County positions.

Responsibilities

• Provide care coordination to individuals eligible for Special Needs Basic Care (SNBC) through UCare; includes assisting members with establishing in-home services, coordinating medical appointments and transportation, helping members navigate complex health systems, assessing for additional resources to meet the person’s needs.
• Provide all required documentation by preparing and maintaining all client records, documenting service and contact activities according to Federal, State Statutes/Rules as well as County policies/guidelines to allow for quality assurance review and maximization of funding.
• Other duties as assigned.
