About The Position

The role of the Inpatient Nurse Liaison is to bridge patients from inpatient hospital services to outpatient clinic services. This RN will provide case management support to both outpatient and inpatient coordination needs. Supporting providers as they manage patient consultations, provide introductory chemotherapy teaches to patients, and coordinate and schedule Inpatient chemotherapy admissions.

Requirements

  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
  • Two years of experience in an acute care nursing that includes experiences in oncology.

Nice To Haves

  • Inpatient & Oncology experience preferred
  • Inpatient experience & case management skills helpful

Responsibilities

  • Oncology RN that will be bridging the gap with patients who are admitted and consulted by our Hematology/Oncology team and the outpatient needs of the clinic.
  • Chemotherapy teaching, setting up admissions for Inpatient Chemo, and scheduling appointments for follow up.
  • Helping to manage/coordinate the care of the consultations by placing and following up with referrals.
