About The Position

This role involves providing home visits for patient assessment, treatment, and support. Responsibilities include pronouncement of patient death, crisis management, and admission. The nurse will develop and revise nursing care plans in conjunction with the patient's physician and the Interdisciplinary Group (IDG). Excellent customer care skills are essential for interacting with internal and external customers, including proper phone etiquette. The position requires preparing clinical and progress notes, informing physicians and other personnel of patient needs and interventions, and evaluating patient/family response to care. Timely submission of all documentation is expected, along with performing other duties as required. Ensuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures is also a key aspect of this role.

Requirements

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State Registered Nurse License
  • Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance

Nice To Haves

  • Two years of experience as an RN preferred in a healthcare/clinical setting

  • Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.
  • Timely submission of all documentation.
  • Performance of other duties as required.
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
