About The Position

The On-Call regi s t e red n urse plans, organizes, and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. ESSENT I AL JOB FUNCTIONS/RESPONSIBILITIES Patient Car e Completes i nitial, comp r ehensive, and ongoing a s sessments o f patient and fami l y t o determine hospice needs. P rovides a c o mplete physical asses s ment and history o f current and previou s il l ness (e s ). Provides professional nursing car e b y util i zing al l element s of the nursing pro c ess. Assesses and evaluate s patient’ s s tatu s b y : Contribute s to the plan of c a r e. Participating i n revis i ng the plan of care a s nece s sar y Uses health a sses s ment data t o determine nursi n g diagnos i s. Contribute s t o th e c a re plan that e s tabl i shes goals, base d on nurs i ng diagnosis and incorporat e s pal l iative nursing a c t i ons. I n cludes the patient and the fami l y in the planning process. Initiate s appropr i ate p re v entive and rehabi l itative nursing procedure s . A d minister s medicat i ons and tre a tm e nts a s pr e scribed b y the physi c ian i n th e physic i an’s plan of care. Counsel s the patient an d fami l y i n m e eting nurs i ng and related nee d s. Adhering to the organiz a tion’s documentatio n an d care proced u re s and s tandards o f personal and p rofessional conduct. Provides healt h care instruction s t o the pat i ent a s appro p riate per a s ses s ment and plan. Assists the patient and/or c a regiver/f a mi l y w ith t h e act i v i t i e s of dai l y l iving, prov i sion and education, and fac i litate s th e patient ’s efforts tow a rd maintaining independence for a s l ong as possible and optional comfort c a re. RN w i ll act as Cas e Manager unti l a C ase Manager is assigned b y Cl i nical Supervisor or designee and assumes responsibi l ity to coordinate patient car e for assigned patient. Communication Completes, maintains, and subm i ts a c cur a te and relevant cl i nical note s regarding patients’ condition and c a re given. Rec o rds pain/sympt o m management changes/ o utcomes a s appropriate. Communicat e s w ith the physician regardin g th e patient’s need s and report s change s i n th e patient’s conditio n ; o b tains/receive s physic i ans’ order s as required. Teaches the p a tient an d fami l y/c a regiver self-car e technique s as ap p ropr i ate. Pr o vides medication, diet and o t h er instru c tions a s ord e red by the physi c ian and r e cognizes and util i zes opportun i ties f o r health counseling w i th p atients and famil i es/car e givers. Work s in concert w i th the interdis ci plinary group. Provides and maintain s a safe enviro n ment f o r t h e patient. Assists the patient and fami l y/caregiv e r and other team m e mbers in p rov i ding continuity o f care. Work s in coop e ration w ith the f a mi l y/caregiv e r and hospice Int e rdi s c i pl i nary Group Members to meet the emotional needs of the pat i ent and f a mi l y/caregiv e r. Addition a l Dutie s Is responsible for all on- c all duties as defined b y the on-cal l poli c y. Ensures that arrangem e nts for equipment and other nece s sar y items and serv i ce s are avai l able. Supervi s es anci l lary per s onnel and delegate s re s pons i bi l ities when required. Requirements

  • Registered nurse w i t h c u rrent licensur e t o pra c t ic e pr o fessional nursing in the s tat e .
  • Graduate of N a tional League f o r Nursing ac c redited schoo l of nurs i ng.
  • Maintains a current CPR certifi c at i on.
  • Must be a li c ensed d river w i th an automobi l e that i s i nsured in accordance with state or organization requireme n ts and is in good w ork i ng order.
  • Minimum of tw o years’ e xperience, a t leas t one o f whi c h i s i n the area o f p ubl i c health, home c a re, or hospic e nursing is pref e rred.
  • Self-directed an d able t o work w i th minimal sup e rv i s i on.
  • Demonstrate s excel l ent observatio n , problem solv i ng, v e rbal and w ritten c ommunic a tions; nursing sk i ll s per c o mpetency check l ist.
  • Shows abi l ity t o organiz e and p rioriti z e w ork l oad i ndependently.

  • Homecare/Homebase/Pointcare experience preferred not required.
