About The Position

Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Requirements

Graduate from an accredited School of Nursing.
Current license to practice as a Registered Professional Nurse in New York State.
2 years of Med/Surg or Telemetry Experience (Highly preferred).

Bachelor's Degree in Nursing.
Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010 and obtain a BSN degree within five (5) years of employment date.
BCLS, ACLS (preferred).

Responsibilities

Assesses the patient's physiologic health status, including gastrointestinal, cardiovascular, respiratory, renal, and neurological systems, as well as mobility, sensory deficits, prostheses use, and skin condition. Determines level of pain and pain management. Communicates and documents patient's physiologic health status and plan of care.
Assesses patient's psychosocial health status, including perception of medical/nursing care, expectations of care, coping mechanisms, knowledge level, and ability to comprehend. Identifies cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction. Communicates and documents psychological status and care plan.
Identifies patient outcomes by developing criteria for measurement and identifying actual/potential patient problems. Assesses patient's need for teaching based on psychosocial and developmental assessment and develops individualized patient goals.
Formulates the plan of care and establishes priorities necessary to achieve expected outcomes. Coordinates the cost-effective use of supplies, equipment, and medication. Documents the plan of care and collaborates with physicians and other health team members.
Implements the plan of care by maintaining vigilance over patient safety, exercising professional skills, reassessing patients as needed, and correctly administering prescribed treatments and using equipment. Applies scientific principles safely, timely, and efficiently. Makes accurate observations and reports/records patient condition and responses to interventions. Acts rapidly and effectively during emergencies. Provides a calm atmosphere and communicates effectively with family/caretakers. Participates in discharge planning and provides discharge instructions.
Evaluates care provided for patient outcomes by measuring effectiveness, performing variance analysis, and designing/implementing/evaluating systems to improve care. Maintains accurate documentation and uses appropriate methods.
Collaborates with other care team members in planning and carrying out treatment regimens, providing direction as needed. Accurately interprets and implements physician's orders. Assists physicians during treatments/procedures. Keeps Patient Care Manager/designee and/or physician informed of patient's condition changes. Uses clinical judgment in delegating assignments to support staff.
Performs grade I-IV Decubitus Care, including preventive skin care measures, applying simple dressings with aseptic technique, and performing irrigations and other skin care procedures.
Administers medications correctly and safely, identifying medication action, dosage, side effects, and implications for the patient. Meets medication administration examination standards. Demonstrates preparation of local solutions and IV drips. Administers and documents medication correctly. Educates patients and significant others related to drug and food interactions.
Participates in patient and family education by explaining conditions, communicating assessment data, recognizing and utilizing teaching opportunities, and providing for early discharge planning. Evaluates the effectiveness of teaching.
Performs related duties as required.
