About The Position

This role involves implementing and tracking customized, evidence-based patient care plans, recording and communicating real-time clinical changes to the care team, coordinating all disciplines to ensure seamless, holistic patient care, precisely carrying out physician and nursing directives with clinical accuracy, and managing discharge planning to ensure safe and successful patient transitions.

Requirements

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.
