About The Position

Ridges Hospital is seeking a passionate and hardworking Registered Nurse to join their Med-Surg/Oncology unit. This 28-bed unit is located in Burnsville, MN, offering a collaborative environment for professional growth and exceptional patient care. The Registered Nurse is responsible for the assessment, planning, delivery, and evaluation of nursing care for assigned patient populations, adhering to all policies, procedures, professional practice guidelines, and the MN Nurse Practice Act. The RN coordinates care with the healthcare team and delegates responsibilities as appropriate. This position is a .6 FTE, working 48 hours per 2-week pay period, with 8-hour shifts, every other weekend rotation (Fri, Sat, Sun), and on the day shift.

Responsibilities

  • Assesses patient needs and identifies expected nursing outcomes, involving the patient, family, and other health team members in gathering data.
  • Uses an ongoing, systematic process to gather relevant information (biological, psychological, spiritual, and social aspects) from patient and family.
  • Analyzes data and information to identify potential risks, prioritize needs and determine priority problems.
  • Identifies outcomes that are age appropriate, culturally appropriate, realistic, and attainable, validating these with patient, family, and other health team members.
  • Documents data and identified outcomes.
  • Develops an individualized nursing plan of care, collaborating with patient, family, and other health team members.
  • Determines nursing interventions based on identified needs and prioritizes them based on patient acuity, available resources, and patient/family preferences and needs.
  • Implements nursing interventions based on the plan of care, coordinating nursing care with all other health team members.
  • Initiates age and culturally appropriate measures to support and meet patient needs while achieving expected outcomes.
  • Initiates patient/family teaching as appropriate to age, readiness to learn and available resources.
  • Delegates direct and indirect patient care activities to appropriate personnel based on needs, condition, potential for risk/harm, complexity of the skill and predictability of the outcome.
  • Anticipates, recognizes, and responds to emergent situations.
  • Evaluates care and patient response to interventions and expected outcomes, modifying interventions based on patient response and changing patient condition.
  • Communicates verbal and written information in a clear, concise, accurate, and timely manner.
  • Collaborates and communicates effectively and respectfully with the patient, family, and members of the health care team within and across care settings to promote partnership-based health care.
  • Demonstrates the ability to advance care by utilizing standards, guidelines, care pathways and continuous improvement to enhance patient outcomes.
  • Identifies gaps in care and practice and brings them forward to applicable person(s).
  • Utilizes improvement methodologies and incorporates data and information into practice to continuously improve or innovate care.
  • Promotes a culture of evidence-based practice (EBP) by participation in relevant activities.
  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.
  • Partners with patient caregiver in care/decision making.
