About The Position

A leading healthcare organization is seeking Registered Nurses to join its Long-Term Acute Care (LTAC) Medical-Surgical team in Peoria, Illinois. The RN will provide comprehensive nursing care to medically complex patients who require an extended hospital stay and continued acute-level treatment. This position involves patient assessment, care planning, medication administration, interdisciplinary coordination, patient and family education, and discharge planning. This is an excellent opportunity for an RN interested in caring for complex medical-surgical patients while developing strong clinical assessment and care-management skills.

Requirements

Graduate of an accredited nursing program with one of the following: Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma
Current Registered Nurse license
Current Basic Life Support certification
Strong clinical assessment and patient-care skills
Knowledge of medication administration and patient safety
Strong written and verbal communication skills
Ability to organize responsibilities, prioritize patient needs, and function effectively under pressure
Basic computer and electronic documentation skills
Ability to work 12-hour night shifts

Nice To Haves

Active Illinois Registered Nurse license
At least six months to one year of Medical-Surgical nursing experience
Previous acute-care hospital experience
Previous LTAC experience
Advanced Cardiovascular Life Support certification
Experience caring for medically complex or high-acuity patients
Qualified new graduates may be considered based on clinical preparation and department needs.

Responsibilities

Provide direct and indirect nursing care using the assessment, planning, intervention, and evaluation process
Complete comprehensive admission assessments and ongoing reassessments
Develop and maintain individualized nursing plans of care
Coordinate care plans with patients, families, physicians, and interdisciplinary team members
Monitor changes in patient condition and communicate concerns promptly to physicians and nursing leadership
Administer medications safely according to established medication-administration standards
Receive and accurately implement physician orders
Assist physicians with examinations, treatments, and specialized procedures
Perform nursing treatments and procedures within the RN scope of practice
Assess and reassess pain and implement appropriate pain-management interventions
Educate patients and families regarding medications, treatments, disease management, and discharge needs
Participate in discharge planning and coordinate referrals to other disciplines
Document assessments, interventions, patient responses, and care plans accurately in the medical record
Supervise and delegate nursing responsibilities based on patient needs and staff competency
Evaluate the effectiveness of nursing interventions and modify plans of care as needed
Provide age-appropriate, culturally sensitive, and patient-centered care
Address psychosocial, cultural, spiritual, and educational needs of patients and families
Maintain strong communication with physicians, nursing leadership, coworkers, patients, and families
Prioritize multiple patient-care responsibilities in a fast-paced clinical environment
Maintain compliance with hospital policies, nursing standards, and patient-safety requirements
