About The Position

Responsibilities matched to your license: RN assignments include nursing assessment, protocol-based telephone triage, care planning and escalation. LPN assignments emphasize information gathering, implementation of care plans, patient education and follow-up under required supervision. The responsibilities and support arrangements are defined according to license, experience and demonstrated competency; the two licenses are not treated as interchangeable. The work: Coordinate transitions after hospital or facility discharge, medication-list review, chronic care follow-up and behavioral health care coordination. Support program consent, accurate documentation and time tracking when required. The purpose is appropriate care and complete follow-through—not merely completing a monthly call. Office and field opportunities: Office-focused, field-focused and blended assignments are considered. Clinical tasks may include patient education, immunizations, specimen collection, wound care or home-safety observations within the agreed role, orders and required supervision. Primarily weekday work; any telephone coverage outside scheduled hours, schedule expectations and compensation are discussed before hire.
