Registered Nurse - Liver Transplant Coordinator
About The Position
The Registered Nurse Coordinator serves as a key clinical resource within the Hepatology/Liver Disease Program, providing comprehensive nursing support for patients with liver disease across the continuum of care. This role is responsible for assisting physicians with patient evaluations, coordinating diagnostic testing and treatment plans, triaging patient concerns, and supporting quality improvement and research initiatives. The RN Coordinator promotes excellent patient outcomes through effective communication, patient education, care coordination, and adherence to Loyola's values of safety, quality, and service excellence.
Requirements
- Bachelor's degree in nursing (BSN)
- Current Registered Nurse (RN) license in the State of Illinois
- Current CPR/BLS Certification
Nice To Haves
- 3 to 5 years of nursing experience in hepatology, gastroenterology, ambulatory care, care coordination, or a related specialty strongly preferred.
Responsibilities
- Assist physicians in the evaluation and management of patients with liver disease.
- Review and communicate diagnostic test results, laboratory findings, and other relevant clinical information to providers.
- Coordinate care plans and ensure timely follow-up for testing, treatment, and referrals.
- Educate patients and families regarding disease processes, treatment plans, medications, and ongoing care needs.
- Triage patient calls related to liver disease management, medications, symptoms, and required testing.
- Assess patient concerns and determine appropriate interventions or escalation to providers.
- Provide clinical guidance and support to patients while ensuring continuity of care.
- Monitor patient needs and facilitate timely communication between patients and the healthcare team.
- Respond to inquiries from CMS, The Joint Commission (JCAHO), insurance companies, physicians, and other healthcare organizations regarding patient management.
- Maintain accurate clinical documentation and support compliance with regulatory requirements.
- Collaborate with interdisciplinary teams to ensure effective patient care coordination.
- Participate in departmental quality improvement initiatives and patient safety programs.
- Support clinical research activities, presentations, and educational projects as assigned.
- Contribute to ongoing efforts to improve patient outcomes, operational effectiveness, and service excellence.
