With deep roots in Western Massachusetts and a growing presence in communities throughout the state, we are currently seeking a Registered Nurse or Licensed Practical Nurse to join our Program of Assertive Community Treatment (PACT) team and help advance our mission of empowering people to live healthier, more fulfilling lives. This is a part-time, 20 hours / week position with flexibility!

Requirements

  • Registered Nurse (RN) licensure or Licensed Practical Nurse (LPN) certification required.
  • Must obtain and maintain MAP certification within introductory period of employment.
  • Valid driver's license and reliable vehicle available for all working hours to transport individuals served required.

Nice To Haves

  • Bachelor’s Degree in related field preferred.
  • Minimum 6 months in Human Service field preferred.

Responsibilities

  • Provide indirect and direct services to individuals served within the PACT program.
  • Collaborate with and provide medical services/coordination to facilitate the care planning process and overall provision of services to the individuals served.
  • Ensure all staff are following MAP Policies and Procedures and follows these procedures.
  • Complete health and medication-related documents as individually indicated and in accordance with MAP regulations.
  • Under the direction of the team Psychiatrist, Nurse Manager, and in collaboration with other Registered Nurses on the team, manage pharmaceuticals and medical supplies.
  • Coordinate, schedule, and administer medical assessments of individual’s physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment.
  • Ensure change of shift log is completed each shift and that change of shift occurs each shift with oncoming staff.
  • Provide safe and timely transportation in own vehicle, as needed for medical appointments, recreational activities, shopping, errands, etc.
  • Provide ongoing information, periodic reviews, and documentation to support treatment and Treatment Plan goals and objectives.
  • Maintain OSHA guidelines.
  • Provide coverage at all PACT locations as needed.
  • Complete individually assigned program-specific responsibilities.
