About The Position

Matrix Home Care is seeking experienced and dependable Registered Nurses and Licensed Practical Nurses to provide skilled nursing services to patients in their homes throughout Volusia County. This opportunity is ideal for nurses with home health experience who are comfortable completing Start of Care assessments and ongoing skilled nursing visits. Candidates should be organized, compassionate, clinically confident, and committed to providing safe, high-quality patient care.

Requirements

  • Current and unrestricted Florida Registered Nurse or Licensed Practical Nurse license
  • Previous home health experience preferred
  • Start of Care and OASIS documentation experience strongly preferred for Registered Nurse applicants
  • Strong clinical assessment, documentation, communication, and time-management skills
  • Current CPR or Basic Life Support certification
  • Valid driver’s license
  • Reliable transportation and current automobile insurance
  • Ability to travel throughout the assigned Volusia County service area
  • Successful completion of an Agency for Health Care Administration Level 2 background screening
  • Ability to work independently in patients’ homes
  • Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.

Responsibilities

  • Complete Start of Care assessments and other assigned home health visits
  • Perform comprehensive patient assessments
  • Provide skilled nursing care according to the physician’s orders and established plan of care
  • Complete routine visits, follow-up visits, resumptions of care, recertifications, and discharge visits, as assigned
  • Provide wound care, medication management, disease-management education, and other skilled nursing services
  • Educate patients and caregivers regarding medications, treatments, safety, and the plan of care
  • Monitor patient progress and report significant changes to the physician and clinical team
  • Coordinate care with physicians, therapists, caregivers, and other interdisciplinary team members
  • Complete accurate and timely clinical documentation
  • Follow all agency policies and applicable state, federal, Medicare, and home health requirements
