About The Position

This role involves coordinating and delivering patient care within an Internal Medicine Clinic, utilizing a health care team philosophy practice model. The Registered Nurse (RN) is responsible for coordinating patient teaching and discharge planning, as well as managing activities with ancillary departments to maintain hospital and departmental standards. The Licensed Practical Nurse (LPN) provides basic therapeutic nursing care under the supervision of an RN, focusing on routine and standardized patient and caregiver needs with predictable outcomes. The LPN assists the RN and healthcare team in achieving desired patient outcomes, relying on education, training, and established guidelines. Initially, the LPN will require instruction and guidance, with the expectation to develop into working under general supervision.

Requirements

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) qualification.
  • Understanding of health care team philosophy practice model.
  • Ability to perform patient teaching and discharge planning.
  • Skills in coordinating activities with ancillary departments.
  • Knowledge of and adherence to departmental and hospital standards.
  • Ability to provide basic therapeutic nursing care.
  • Ability to work under the direction and supervision of an RN.

Responsibilities

  • Coordinates and delivers patient care using the health care team philosophy practice model.
