About The Position

The Registered Nurse (RN) or Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care to patients in a skilled nursing facility or similar healthcare setting. This position involves comprehensive patient assessment, development and implementation of individualized care plans, and continuous monitoring of patient progress to ensure optimal health outcomes. The RN collaborates closely with interdisciplinary healthcare teams, including physicians, therapists, and social workers, to coordinate care and facilitate patient recovery. Additionally, the role requires educating patients and their families about health conditions, treatments, and post-discharge care to promote long-term wellness. Ultimately, the RN is responsible for maintaining a safe, compassionate, and therapeutic environment that supports patient dignity and quality of life.

Requirements

• Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Georgia
• Graduation from an accredited nursing program (Associate's or bachelor's degree in nursing).
• Basic Life Support (BLS) certification.
• Minimum of one year of clinical nursing experience, preferably in a skilled nursing or long-term care setting.
• Strong knowledge of nursing principles, patient care standards, and regulatory requirements.
• Ability to write reports, business correspondence, procedure manuals.
• Ability to communicate in courteous, friendly manner and effectively present information and respond to questions from managers, clients, customers, and the general public.
• Ability to effectively manage and direct subordinates.
• Ability to calculate figures related to medication dosages, and amounts such as percentages and area.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Conduct comprehensive resident assessments upon admission and throughout care to evaluate health status, identify needs, and monitor changes in condition.
• Develop, implement, update, and monitor individualized care plans in accordance with physician orders, nursing standards, and resident needs.
• Administer prescribed medications and treatments accurately while monitoring effectiveness, adverse reactions, and special care needs.
• Provide direct nursing care and assistance with Activities of Daily Living (ADLs) while promoting residents’ independence, dignity, and quality of life.
• Coordinate resident care with physicians, nursing staff, rehabilitation services, social services, and other interdisciplinary team members.
• Respond promptly and professionally to resident call systems, emergencies, incidents, and changes in resident conditions, notifying physicians and responsible parties as appropriate.
• Make rounds with physicians, assist with examinations and treatments, and carry out medical orders as directed.
• Maintain accurate, complete, and timely documentation of nursing care, assessments, medications, incidents, resident progress, and required regulatory records.
• Educate residents and families regarding medical conditions, medications, treatment plans, rehabilitation, and wellness strategies.
• Ensure compliance with infection control practices, safety standards, resident rights, facility policies, and all applicable regulatory requirements.
• Monitor the resident environment to maintain safety, sanitation, and quality standards throughout the facility.
• Participate in admissions, resident orientation, emergency preparedness, quality improvement initiatives, and staff education programs.
• Provide daily supervision, leadership, guidance, and support to assigned staff to ensure quality resident care and effective team performance.
• Assist with staff training, evaluations, coaching, and corrective actions as assigned.
• Demonstrate professionalism, sound clinical judgment, hospitality, and the core values of quality, respect, service, and integrity in all interactions.
• Communicate regularly with the Director of Nursing regarding resident conditions, staffing concerns, facility needs, and nursing supply requirements.
• Perform all other duties as assigned by supervisor.
