About The Position

Horizon Health in Huron, South Dakota, is seeking a full-time Registered Nurse, Licensed Practical Nurse, or Certified Medical Assistant to provide nursing care to patients in an outpatient clinic setting. The role supports high-quality, patient-centered care in a collaborative, team-oriented environment focused on individual patient needs and community health and wellness.

The position coordinates care throughout each patient visit, provides patient education and care coordination, and performs nursing skills within the scope of the employee’s license or certification. On-the-job training is provided for labs and x-rays.

Requirements

• Registered Nurses and Licensed Practical Nurses must have a valid South Dakota nursing license.
• Medical Assistants must be certified through an accredited program.

Responsibilities

• Coordinate care throughout the patient visit, including rooming patients, taking vital signs, collecting an adequate history, and updating medications.
• Update patient clinical information in the electronic health record.
• Prepare patients for provider visits, schedule follow-up care as needed, and clean and stock the room.
• Provide patient education about overall health management.
• Initiate referral management and coordinate care for preventative and specialty visits.
• Develop care plans, including self-management plans and goals.
• Initiate and perform nursing skills within the scope of the employee’s license, including provider-ordered injections and vaccinations.
• Perform labs, x-rays, and other tests; on-the-job training is provided for labs and x-rays.
