Old Glory Home Health Care & Assistive Care is seeking compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join their home health team in Central Virginia. This clinician-led, physician- and nurse-owned agency focuses on providing supported, respected, and proud care to patients in their homes. Requirements
- Active Virginia RN or LPN license
- Current CPR certification
- Reliable transportation
- Valid driver’s license and auto insurance

- OASIS experience (for RN applicants)
Responsibilities
- Provide skilled nursing visits in the patient’s home.
- Perform patient assessments.
- Coordinate care.
- Provide medication education.
- Perform wound care.
- Manage chronic diseases.
- Provide post-hospital follow-up.
- Support patients recovering from illness, surgery, injury, or a change in condition.
- Conduct start of care, recertification, resumption of care, and discharge assessments (RNs).
- Perform OASIS documentation and care planning (RNs).
- Perform medication reconciliation (RNs).
- Manage chronic disease (RNs).
- Communicate with physicians, families, therapists, and the care team (RNs).
- Help patients remain safe, independent, and confident at home.
- Provide hands-on skilled nursing care under the direction of the RN and physician plan of care (LPNs).
- Perform medication education and treatment follow-up (LPNs).
- Monitor vital signs (LPNs).
- Support for chronic disease management (LPNs).
- Document patient progress and changes in condition (LPNs).
- Communicate with the RN case manager and clinical leadership team (LPNs).
- Help patients stay safe and supported at home (LPNs).
