About The Position

Requirements

  • Active Virginia RN or LPN license
  • Current CPR certification
  • Reliable transportation

Responsibilities

  • Provide skilled nursing visits in the patient’s home.
  • Perform patient assessments.
  • Coordinate care.
  • Provide medication education.
  • Perform wound care.
  • Manage chronic diseases.
  • Provide post-hospital follow-up.
  • Support patients recovering from illness, surgery, injury, or a change in condition.
  • Conduct start of care, recertification, resumption of care, and discharge assessments (RNs).
  • Perform OASIS documentation and care planning (RNs).
  • Perform medication reconciliation (RNs).
  • Manage chronic disease (RNs).
  • Communicate with physicians, families, therapists, and the care team (RNs).
  • Help patients remain safe, independent, and confident at home.
  • Provide hands-on skilled nursing care under the direction of the RN and physician plan of care (LPNs).
  • Perform medication education and treatment follow-up (LPNs).
  • Monitor vital signs (LPNs).
  • Support for chronic disease management (LPNs).
  • Document patient progress and changes in condition (LPNs).
  • Communicate with the RN case manager and clinical leadership team (LPNs).
  • Help patients stay safe and supported at home (LPNs).
