We're hiring for a Full-Time (1.0 FTE) RN at our SL Kidney and Hypertension Clinic! Join a collaborative team where RNs are an integral part of supporting patients with kidney disease and hypertension. In this role, you’ll have the opportunity to build relationships with patients while coordinating their ongoing care.

Requirements

  • Nursing Degree
  • 1 years’ experience
  • Current RN licensure from the State of Practice
  • Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.

Nice To Haves

  • Previous experience in triage or telehealth role is preferred
  • Dialysis or nephrology experience is a plus
  • Strong clinical assessment and critical thinking skills, excellent verbal and documentation skills, ability to work independently

Responsibilities

  • Triaging clinical concerns and communicating with patients about their care needs.
  • Coordinating care and collaborating closely with nephrology providers.
  • Managing medications and supporting patient medication assistance needs.
  • Providing patient education, follow-up, and ongoing communication.
  • Supporting specialty programs and services.
  • Coordinating 24-hour blood pressure monitoring, including device placement.
