About The Position

The Massachusetts Mental Health Center, a part of the Commonwealth of Massachusetts’ Department of Mental Health is seeking a dynamic Registered Nurse IV, to oversee the daily operations of nursing staff and services at the Fenwood Inn. The RN IV acts as licensed supervisor for his/her shift providing clinical oversight, supervision, intakes, assessments/evaluations and implementation of emergency procedures. This position also provides medication teaching and oversees and documents self-administration of medication and shift activities of 40 adult clients and performs charge/supervisory duties for all shift staff and Mental Health Workers. Note: The hours for this position are Monday through Friday 2:30pm to 11pm. Employees in this position will be required to perform mandatory overtime.

Requirements

  • Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing
  • Two years of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility
  • Of which at least one year must have been in a supervisory, administrative or managerial capacity
  • Knowledge of the principles of physiological, psychological and emotional growth and development.
  • Familiar with the types and characteristics of drugs used in treating physical and mental disorders.
  • Aware of and uses of diagnostic procedures employed in nursing practice.
  • Experienced utilizing interviewing and problem-solving techniques.
  • Knowledge of psychotherapies and principles of psychosocial rehabilitation.
  • Ability to understand and apply rules, regulations, and laws governing agency and unit and maintain accurate records.
  • Adept at following and giving clear and precise oral and written instructions.
  • Ability to read and interpret documents such as physician’s orders and treatment plans as well as participate in a multi-disciplinary health care team.
  • Able to exercise sound judgement, make decisions, and act quickly in emergency situations.
  • Skilled in establishing rapport and dealing effectively with persons from diverse cultures, races and lifestyles.
  • Ability to maintain confidentiality of client information.
  • Accomplished motivating clients to participate in individual treatment plans.
  • Experienced treating and interacting with persons who are under physical/emotional stress.
  • Proficient assessing, analyzing, drawing conclusions, and determining the applicability of clinical data in order to make and/or take appropriate actions, recommendations, or referrals.
  • Able to supervise and evaluate staff and trainees as well as plan and conduct nursing trainings.
  • Skilled in preparing and implementing treatment plans.

Responsibilities

  • Provides direct nursing care including medication administration, taking vital signs, doing scheduled treatments and counseling the patients.
  • Assesses and evaluates each client daily for any psychiatric or medical changes.
  • Provides daily medication teaching to clients of the Fenwood Inn to help them become medication independent.
  • Develops and maintains appropriate therapeutic relationships with clients demonstrating adaptability in response to client preferences and changing needs.
  • Provides clinical oversight for clients and staff regarding all clinical and medication issues.
  • Communicates with clinicians on clinically significant issues or changes and documents in shift report and client chart.
  • Performs all charge/supervisory responsibilities for all staff (MHW’s) for his/her assigned shift.
  • Provides liaison and consultation with on-call CCS physician regarding client issues, both psychiatric and medical.
  • Provides emergency assessment and evaluation of a client in crisis, develops a crisis intervention plan with staff, and, if necessary, consults with the B.E.S.T. team if he/she deems a client in need of a B.E.S.T. evaluation.
  • Oversees and documents self-medication of 40 adult clients.
  • Monitors/updates physician orders each shift and performs monthly order summary duties.
  • Identifies staff training needs and provides programming to meet educational needs.
  • Communicates to administration on all critical incidents including clients missing from Fenwood Inn for 24 hours.
  • Responsible for updating and completing registration forms on client admission to and discharge from the Fenwood Inn.
  • Facilitates implementation of Rehab option treatment plan with focus on medication compliance goals for identified Fenwood Inn clients.
  • Acts as code blue and code green coordinator for his/her shift.
  • Implements treatment plans using sound clinical judgement, demonstrated through the use of initiative, good decision making ability and follow through with goals/plans.
  • Collaborates with other departments of MMHC, DMH, and other agencies community programs and social services.
  • Provides case management services to facilitate least restrictive care, and promotes the highest level of functioning for clients in the community.
  • Communicates with clients, families, significant others, and other professionals to advocate on behalf of the clients’ treatment goals.
  • Responds positively to changes in clinical priorities.
  • Adapts positively to changes in response to change in procedure, responsibility, or assignment at the Fenwood Inn.
  • Is punctual, and present at start of shift and monitors MHW punctuality.
  • Recommend new equipment purchases.
  • Seeks additional knowledge and skills to promote growth by attending conferences, giving conferences, etc.
  • Provides supervision to other staff and trainees as assigned.
  • Completes paperwork in a timely manner according to DMH, MMHC and third-party regulatory standards for quality assurance, utilization review and legal requirements.
