LifeLong Medical Care has an exciting opportunity for a RN IV Nurse Supervisor to join our multi-disciplinary team providing primary care services at our Brookside San Pablo Health Center. The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, and experience working with behavioral health diagnoses. Under the general supervision of the Regional Medical Director and Center Director, with clinical guidance of the Sr Director of Clinical Care Teams, the Nurse Supervisor is responsible for providing clinical care, administrative structure and overseeing the work of Registered Nurses and Medical Assistants within the health center. The nurse will participate in telephonic and in person patient triage, education and counseling, participating in the ongoing education of the care team and collaborating with medical providers to maximize quality of care for our patient populations as well as nurture professional development of RN’s and MA’s.
