About The Position

Provide nursing care requiring specialized knowledge, judgement, and skill derived from the principles of biological, physical, behavioral, social, and nursing sciences.

Requirements

  • Active and unrestricted Registered Nurse license and certification
  • Completion of all necessary continuing education to maintain licensing/certification
  • Adherence to current HIPAA regulations and federal/state laws for patient protected health information (PHI) and/or medical record
  • Adherence of all AHP policies/procedures as they pertain to patient PHI and the medical record
  • Maintain strict confidentiality of all patient information
  • Nursing knowledge and judgement
  • Ability to administer vaccines, medications, treatments, and regimens prescribed by the physician
  • Ability to perform laboratory tests, determinations, and cultures according to procedures
  • Ability to prepare and document prescription refills and medical equipment and services refills for physician approval
  • Ability to schedule tests, procedures, consultations, and clinical evaluations
  • Ability to accurately report and document all nursing advice, vaccines, medications, treatments, laboratory tests, and scheduled procedures
  • Ability to monitor assigned ADHD, asthma, and mental health patients
  • Ability to ensure appropriate follow-up, visits, labs, tests/imaging, consults, etc. are scheduled as needed and/or as ordered by the physician
  • Ability to participate in daily care team huddles
  • Ability to participate in quality improvement initiatives
  • Completion of necessary training on topics including, but not limited to, care coordination, patient self-management, population management, and health literacy
  • Ability to attend monthly staff meetings and clinical department meetings

Responsibilities

  • Maintain active and unrestricted Registered Nurse license and certification; complete all necessary continuing education to maintain licensing/certification
  • Adherence to current HIPAA regulations and federal/state laws for patient protected health information (PHI) and/or medical record; adherence of all AHP policies/procedures as they pertain to patient PHI and the medical record; maintain strict confidentiality of all patient information
  • Triage patients/families by telephone or in person according to medical priority
  • Assess health status and provide health counseling and teaching using nursing knowledge and judgement
  • Administer vaccines, medications, treatments, and regimens prescribed by the physician
  • Perform laboratory tests, determinations, and cultures according to procedures prescribed by the manufacturers and the Laboratory Director
  • Prepare and document prescription refills and medical equipment and services refills for the physician to approve
  • Schedule tests, procedures, consultations, and clinical evaluations to be done by ancillary medical service facilities
  • Accurately report and document all nursing advice given; all vaccines, medications and medication reconciliation, and treatments given; all laboratory tests, determinations, and culture performed; all tests, procedures, consultations, and clinical evaluations scheduled at ancillary medical service facilities in a timely manner
  • Monitor assigned ADHD, asthma, and mental health patients; ensure that all appropriate follow-up, visits, labs, tests/imaging, consults, etc. are scheduled as needed and/or as ordered by the physician
  • Participate in daily care team huddles to ensure appropriate and quality patient care
  • Participate in quality improvement initiatives as needed
  • Complete necessary training on topics including, but not limited to, care coordination, patient self-management, population management, and health literacy
  • Attend monthly staff meetings and clinical department meetings
  • Other nursing and non-nursing duties that may be identified from time to time
