About The Position

This position is for a Registered Nurse II in Pediatrics, focusing on clinic care coordination. The role involves assisting clinicians with patient care, including taking vital signs, preparing immunizations, performing dressing changes, pulse oximetry, and respiratory treatments. The nurse will accurately document findings, plans, and interventions in the medical record, evaluating the effectiveness of care and making necessary adjustments. A key aspect of the role is assessing patients' physical, psychosocial, and developmental needs, preparing charts, and establishing or reviewing care plans. The nurse will also provide patient and family education, utilizing appropriate tools and communication styles, and evaluating the effectiveness of the teaching plan. Documentation responsibilities include contacts, chart notes, and other data in various systems such as Immunization Databases, Provide Case Management CMS, Specialty SNAQ Reports, and Epic. The position involves monitoring the quality of practice within the HIV-infected/exposed patient population through participation in a continuous quality improvement program. The nurse will act as a member of the healthcare team, serving as a liaison between patients, families, and the clinical team to ensure quality customer service. They will also act as a resource for peers and inpatient staff, demonstrating clinical expertise for specific populations, and attending educational activities and reviewing relevant literature. The role includes performing phlebotomy, HIV testing, and other assigned labs under Universal Precautions. The nurse will recognize the need for referrals to sub-specialists or other agencies and facilitate these processes, coordinating with outside pharmacies, patient hospital discharge planning, and specialists. Assuring continuity of care through collaboration with other healthcare providers, community agencies, and institutions is essential. Participation in multi-disciplinary staffing, discharge staffing, and family conferences is expected. The nurse will also participate in relevant nursing and specialty organizations, as well as research, department, and hospital-wide meetings. Some minimum travel between clinics is required.

Requirements

  • Graduate of a nationally accredited nursing program
  • One year of professional nursing experience
  • Licensed as a registered nurse in accordance with Chapter 464, Florida Statutes
  • Level II Background required

Responsibilities

  • Assists the clinician in delivering patient care including taking vital signs, preparing immunizations, Dressing changes, pulse oximetry and respiratory treatments and other required procedures.
  • Accurately and efficiently documents findings, plans and interventions in the medical record.
  • Evaluates the effectiveness of care and documents subsequent changes to plan or follow up on medical record.
  • Assesses individual physical, psychosocial and developmental needs of patients.
  • Preprocesses and post processes the patient chart for each clinic visit.
  • Establishes or reviews plan of care and modifies as needed based on assessment.
  • Implements the established plan of care competently and consistently.
  • Demonstrates advanced clinical and technical skills and can accurately assess and intervene when deviations from expected outcomes occur.
  • Identifies specific education needs for patients/families.
  • Accesses and utilizes appropriate teaching tools and techniques to facilitate patient/family education.
  • Utilizes appropriate style and level of communication.
  • Evaluates and documents effectiveness of teaching plan.
  • Responsible for documenting contacts, chart notes and other required data in medical record and the pertinent information systems used by UF CARES including Immunization Databases, Provide Case Management CMS, Specialty SNAQ Reports, Epic and other systems as required.
