This role is for a Registered Nurse II within a Home Health Agency, working part-time with 24 weekly hours and a 0.6 FTE. The position is considered regular employee status. The nurse will follow professional practice standards, assist in developing competence in peers, and utilize the nursing process for safe and ethical practice. They will also be involved in identifying and implementing safety best practices, mitigating risks, and supporting research studies. The role requires systems thinking to understand and improve the patient experience within the healthcare environment, anticipating patient and family needs. Inclusion is a key aspect, involving consideration of patient values, cultural differences, and accommodating personal and professional differences in the plan of care. The nurse will also engage the patient's family and protect their rights. Clinical judgment involves collecting and interpreting complex patient data, making judgments based on the overall picture, and responding to subtle changes. Documentation and teaching others to document the plan of care, care delivered, and outcomes are essential. The nurse will engage with patients and families compassionately, tailor caring practices, promote a healing environment, and ensure safety throughout transitions. Integrating age-specific concepts and supporting patient/family adaptation to the home environment are also key responsibilities. The nurse will use self-management support and teach-back strategies to evaluate effectiveness and coordinate education with the healthcare team.
