About The Position

Deliver compassionate, high-quality direct patient care that strictly adheres to established organizational protocols and best practices. Manage the entire patient care journey, meticulously documenting progress, executing physician and nursing orders promptly, and communicating critical status updates to the multidisciplinary team to ensure continuity of care. Coordinate care across all clinical disciplines, acting as the primary liaison to guarantee seamless and well-integrated patient services and ensure optimal care outcomes. Facilitate comprehensive discharge planning by assessing patient needs and collaborating with the healthcare team to ensure a safe, smooth, and successful transition home.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or Multi-State Compact obtained prior to hire date or job transfer date required. Additional certifications may be required for specific job locations.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
