Department: CC207688 IMED - Hialeah West Job Description Summary: manage an assigned panel of patients in the Primary Care practices. Coordinate all aspects of care, outpatient care, post acute care and ancillary testing. Manage high risk patients between office visits to ensure adherence. Assist with calling patients with results/questions/concerns about medications, assist physicians with in clinic procedure, work on physicians clinical inbasket and serve as the liaison between patients and provider/extender.
