About The Position

Virtua Health is introducing an innovative "Hospital from Home" (H@H) program, a technology-driven care model that allows patients to receive medical-surgical level care safely in their homes. This program offers a more personal interaction for nurses compared to traditional floor settings. Patients are carefully selected based on criteria set by physicians, nurses, and administrators, either during an existing admission or directly from the Emergency Room. The H@H program enables patients to recover in their familiar environment. Nurses in this program will make two visits per day (or more as needed) to assess patients, administer medications, provide education, and perform other nursing interventions. Mileage and necessary equipment will be provided, and Durable Medical Equipment (DME) will be sent to the home prior to discharge. This is an on-site position requiring travel within the surrounding communities of Virtua's five acute care hospitals, with some work also performed at the Hospital at Home Command Center.

Requirements

  • Minimum of 2-3 years of acute care nursing experience.
  • Current State of New Jersey nursing license to practice as a registered nurse.
  • Current CPR certification and BLS.
  • Must have current proof of valid driver's license and auto insurance.

Nice To Haves

  • Experience working in an emergency department setting or home health.
  • Excellent assessment skills and clinical judgment.
  • BSN preferred.

Responsibilities

  • Perform and document the nursing process – assessment, diagnosis, planning, implementation, and evaluation of patients receiving care remotely or in-person at their defined home.
  • Review and follow-up on patient education, implemented patient care plans, and coordinated services.
  • Triage, identify, and act on changes in patient condition.
  • Utilize and instruct patient and family on the use of the telehealth tablet used for biometric data capture, patient symptom reporting, and telehealth visits.
  • Adhere to policies and procedures of the organization, including HIPAA and Patient confidentiality regulations.
  • Maintain nursing supplies and anticipate and restock needed supplies.
  • Maintain a safe and clean working environment.
  • Perform all other duties as assigned.
