About The Position
Phoenix Home Care & Hospice is a mission-driven company committed to supporting its employees through the challenges of the healthcare industry. The Phoenix Hospice Registered Nurse provides compassionate nursing care to clients and their families facing a terminal diagnosis. This role involves working with an interdisciplinary team to create and implement a patient/family-centered plan of care, including discussions about goals of care and guidance on healthcare decisions to ensure quality of life.
Requirements
- Kansas or Compact Nursing License
- Strong organizational and self-management skills
- Valid driver’s license
Nice To Haves
- Previous hospice experience preferred.
Responsibilities
- Skilled Visits
- Day-to-day patient interaction
- Education on Health Conditions, such as CHF, CODP, Diabetes
- Wound Care
- Medication Management
- Changes dressings, bandages and contraptions as prescribed
- Documentation of patient’s condition and pointing out significant changes, to be reported to the physician.
- Working alongside other nurses, facility staff, physicians, intake coordinators, and hospice consultants
