About The Position

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice care team. As a Hospice Nurse, you will assume responsibility and accountability for assessing, planning, implementing, and evaluating individualized care plans. You will utilize the nursing process, therapeutic interventions, evidence-based practices, and seamless coordination with an interdisciplinary team of healthcare professionals. Requirements

  • Current RN License in state of practice
  • Basic Life Support (BLS) Certification for healthcare providers.
  • Current driver’s license, insured and reliable transportation and an acceptable driving record
  • Ability to communicate effectively both verbally and in writing
  • Bachelor's Degree in Nursing (BSN) is required as of the job description's last update on 4/27/2025.
  • RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within 4 years of their hire date.

Nice To Haves

  • Previous experience in an Acute Care, Home Health, or Hospice setting.

Responsibilities

  • Assume responsibility and accountability for assessing, planning, implementing, and evaluating individualized care plans.
  • Utilize the nursing process, therapeutic interventions, evidence-based practices, and seamless coordination with an interdisciplinary team of healthcare professionals.
  • Deliver compassionate care to patients in their own homes.
  • Manage patient care autonomously while receiving support from a collaborative healthcare team.
  • Provide specialized experience in areas such as wound care, medication management, and IV therapy, all in a homecare setting.
