About The Position

Eventide Hospice in York, NE is seeking a Hospice Registered Nurse to work 8a-5p Monday to Friday, with occasional weekends and holidays. The Hospice RN is responsible for planning, organizing, and directing hospice care for Eventide Hospice Seniors/residents/clients. This role involves comprehensive assessments, developing and implementing care plans, administering medications and treatments, providing patient education, assisting with daily living and comfort care, and acting as a Case Manager when assigned. Accurate and timely documentation of patient status and care is essential, along with effective communication with physicians regarding patient needs and condition changes.

Requirements

At least age 18.
Successful completion of a Registered Nurse course of study.
RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required.
CPR required.
Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required.
Must possess auto insurance at or above Nebraska statutory requirements.
Must meet Eventide’s vaccination requirements.

Nice To Haves

Bachelor’s degree in Nursing preferred.
Two years of acute care/long term care nursing experience within the last five years preferred.
Experience in community health and/or hospice preferred.

Responsibilities

Completes an initial, comprehensive and ongoing comprehensive assessment of Senior/client and family to determine hospice needs.
Provides a complete physical assessment and history of current and previous illness(es).
Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
Assesses and evaluates Senior’s/client’s status by writing and initiating plan of care that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Regularly re-evaluates Senior/client and family/caregiver needs.
Participates in revising the plan of care as necessary, including the Senior/client and the family in the planning process.
Uses health assessment data to determine nursing diagnosis.
Initiates appropriate preventative nursing procedures.
Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
Provides health care instructions to the Senior/client as appropriate per assessment and plan.
Assists the Senior/client with the activities of daily living and comfort care.
Acts as Case Manager when assigned by Patient Care Coordinator and assumes responsibility to coordinate Senior/client care for assigned caseload.
Completes, maintains and submits accurate and relevant clinical notes regarding Senior’s/client’s condition and care given.
Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the Senior’s/client’s needs and reports changes in the Senior’s/client’s condition; obtains/receives physicians’ orders as required.
