About The Position

Park Nicollet Hospice is looking to hire a Referral and Scheduling Registered Nurse to join our team! The Referral and Scheduling RN is essential to the seamless coordination of referrals and scheduling across Home and Community Services, supporting a broad range of programs including hospice, home care, palliative care, and other specialized community services. This role manages the referral through admission, and scheduling processes ensuring patient visits are scheduled appropriately to maintain manageable caseloads and high-quality care. The RN collaborates closely with interdisciplinary teams, coordinates coverage when team members are unavailable or require visit support, and serves as a key resource for clinical, reimbursement, and regulatory matters. Compassionate communication, strong organizational skills, and a commitment to supporting patients, families, and staff are central to success in this role. This role will focus primarily on supporting Hospice referral and scheduling.

Requirements

  • Bachelor’s degree in nursing
  • 2+ years of hospice, palliative care, home care, and other community care service experience
  • MN or WI Registered Nurse license
  • Excellent verbal and written communication skills.
  • Strong organizational skills.
  • Strong critical thinking, analytical and problem-solving skills.
  • Ability to work independently
  • Ability to work effectively with change
  • Strong computer and technology skills

Nice To Haves

  • 5+ years hospice, palliative care, home care, and other community care service experience
  • MN and WI RN Licensure
  • Experience with the EPIC EMR setting
