The RN Case Manager performs admissions and administers nursing care for patients of all ages in their place of residence. They coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency and assume responsibility for coordination of care. The role involves assessing hospice patients, developing and documenting care plans, providing nursing care with infection control measures, and working with the Interdisciplinary Group/Team to ensure patient/family needs are met. The RN will communicate with physicians, submit required paperwork timely, participate in hospice-sponsored in-service programs and the QAPI program, promote the Hospice’s philosophy and administrative policies, meet productivity standards, and perform on-call responsibilities. Effective communication with patients, families, team members, and other healthcare professionals is essential. Reliable transportation and valid auto liability insurance are required.
