About The Position

We are currently seeking a Registered Nurse (RN) specializing in Hospice Nursing and Case Management to join our team. In this role, you will provide comprehensive case management services to hospice patients, ensuring that they receive compassionate, quality care aligned with their needs and wishes. Your expertise will assist patients and their families through end-of-life care, coordinating services and advocating for patient-centered care plans. Requirements

• Valid and current Registered Nurse (RN) license.
• Minimum of 2 years nursing experience, with at least 1 year in hospice care and case management preferred.
• Strong clinical assessment and care coordination skills.
• Excellent communication and interpersonal skills to effectively work with patients, families, and multidisciplinary teams.
• Knowledge of hospice regulations, ethical considerations, and end-of-life care best practices.
• Ability to advocate for patients and manage complex care situations compassionately.

Responsibilities

• Perform comprehensive assessments of hospice patients to develop individualized care plans that address physical, emotional, and psychosocial needs.
• Coordinate care among interdisciplinary teams, including physicians, social workers, and chaplains.
• Provide case management services by monitoring patient progress, adjusting care plans, and facilitating access to necessary resources and services.
• Educate patients and families regarding hospice care, symptom management, and available support services.
• Maintain accurate, thorough documentation of care provided, patient status, and communications in electronic health records.
• Advocate for patients’ rights and preferences to ensure quality end-of-life care aligned with their values.
• Stay current with standards of hospice care and participate in ongoing professional development.
