As a Hospice Nurse, you’ll play a vital role in ensuring patients receive exceptional care in the setting they cherish most—their home. We're looking for a dynamic and compassionate RN Case Manager to join our Home Health team. In this vital role, you’ll lead the coordination and delivery of exceptional patient care within the home setting—serving as a central figure on our interdisciplinary team. You’ll take ownership of the entire patient care journey, from admission through discharge, ensuring seamless communication, high-quality outcomes, and efficient resource use. Your leadership will guide planning, education, and collaboration across disciplines, while fostering strong relationships with patients, families, and payors.

Requirements

  • Current RN License in state of practice.
  • Basic Life Support (BLS) Certification for healthcare providers.
  • Current driver’s license, insured and reliable transportation and an acceptable driving record.
  • Ability to communicate effectively both verbally and in writing.
  • RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within 4 years of their hire date.

Nice To Haves

  • Bachelor's Degree in Nursing (BSN)
  • Previous experience in an Acute Care, Home Health, or Hospice setting.
