As a Hospice Admission Registered Nurse / RN, you will enable patients to spend quality time with their loved ones and doing the activities they enjoy. You will assess patients for hospice eligibility, explain hospice care services to patients, families/caregivers, ensure all patient’s initial needs are addressed, and collaborate with Inter-Disciplinary Team members regarding the initial plan of care, patient’s condition, and other essential information.

Requirements

• RN license in the state you work
• Two years of experience as a registered nurse
• Current driver’s license
• Current CPR and BCLS certification

• hospice experience preferred

Responsibilities
