About The Position

Haven Hospice is seeking a passionate, dedicated Hospice Admissions RN to join our team in Palatka, FL. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care.
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
