About The Position

The Registered Nurse (RN) with Home Health is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of Home Health patients and their families in their home setting. The RN provides the best clinical practice in an environment of caring and healing while working autonomously in the home. Utilizing the nursing process, the RN delivers care in compliance with hospice/home health policies and procedures, legal and regulatory standards. The RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice, and coordination of care with other health team members. Requirements

  • Current RN License in the state of practice
  • Basic Life Support (BLS) certification for healthcare providers
  • Valid driver’s license with reliable transportation, acceptable driving record, and current auto insurance
  • Bachelor’s Degree in Nursing (BSN)
  • Effective Communication Skills – Ability to communicate clearly and professionally with patients, families, and healthcare teams
  • RNs with less than 12 months of experience as an RN must complete their BSN within five years of their start date.
  • Bachelor of Science in Nursing (BSN) is required as of the job description's last update on 4/27/2025.

Nice To Haves

  • One year of RN experience in Acute Care, Home Health, or Hospice settings

Responsibilities

  • Assessing, planning, implementing and evaluating care of Home Health patients and their families in their home setting.
  • Providing the best clinical practice in an environment of caring and healing while working autonomously in the home.
  • Delivering care in compliance with hospice/home health policies and procedures, legal and regulatory standards.
  • Utilizing the nursing process by use of technology, therapeutic intervention, evidence-based practice, and coordination of care with other health team members.
  • Enhancing career by learning unique skills such as wound care, injection and medication administration, IV monitoring, and patient/caregiver education.
  • Managing patient care while receiving support from a collaborative healthcare team.
  • Delivering compassionate care to patients in their own homes.
  • Seeing and reading information, labels, assessing patient needs, operating monitors, identifying equipment and supplies.
  • Interacting with patient care providers, patients, and visitors, verbally communicating as well as hearing and understanding spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manipulating complex and delicate equipment with precision and accuracy.
  • Computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.
  • Lifting and utilizing full range of movement to transfer patients.
  • Bending to retrieve, lift, and carry supplies and equipment.
  • Walking and assisting with transporting/ambulating patients and obtaining and distributing supplies and equipment.
  • Pushing/pulling portable equipment, including heavy items.
  • Navigating crowded and busy rooms (full of furniture, equipment, power cords on the floor, etc).
  • Ascending and descending stairs or uneven surface in order to access patients.
  • Driving a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
