About The Position
The RN Field Nurse / Registered Nurse, Home Health position is a PRN role located in Hapeville, covering Fayette, Fulton, Clayton, and Fulton counties. This role is not remote and does not involve on-call duties. The salary range is $85,000-$93,000 based on experience. The primary responsibility is to maintain the delivery of quality patient care by ensuring the Patient Care Plan is followed. This role involves direct and indirect patient care for pediatric, adolescent, adult, and geriatric patients. The Staff Nurse will direct CHHAs and CNAs, perform patient assessments, develop and evaluate patient care plans, and document all care provided.
Requirements
- Registered Nurse with current licensure to practice nursing in the state.
- Registered Nurse with minimum (1) year of experience.
- Possesses and maintains current CPR certification.
Responsibilities
- Assessment of the status of nursing needs of assigned patients, implements the individualized patient care plan, evaluates the effectiveness of the care plan, and modifies as appropriate.
- Responsible for promoting and maintaining optimal client care. Diagnosing and providing treatment for human responses to actual or potential health problems.
- Makes the initial nursing evaluation visit.
- Re-evaluates the client's nursing needs as required.
- Initiates the plan of care and necessary revisions.
- Provides those services requiring substantial special skills within nurses scope of Practice Act.
- Assigns, supervises and teaches other clinical personnel, including written para-professional orders and supervision.
- Initiates appropriate preventative and rehabilitative nursing procedures.
- Informs the physician and other personnel of changes in the client's condition and needs.
- Counsels and instructs the client and family in meeting nursing and related needs.
