About The Position

This role involves providing direct patient care in a home health setting. The Registered Nurse will perform assessments, acute triage, wound care, and implement individualized care plans. They will also coordinate care with an interdisciplinary team, monitor patient status, carry out physician orders, and support discharge planning. As a PRN Field Nurse, the role requires supporting patient care coverage and staffing needs, including weekday/weekend shifts, after-hours on-call rotation, and travel to participants' homes, long-term care, and assisted living facilities. A minimum of 4 hours per scheduled shift is expected, with additional hours based on census and operational needs.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Responsibilities

Provide direct patient care, including assessments, acute triage, blood draws, wound care, and implementation of individualized care plans in accordance with established clinical standards.
Coordinate and communicate patient care across the interdisciplinary team, monitor patient status, carry out physician orders, and support discharge planning for seamless care transitions.
Participate in care plan development, team meetings, and patient/caregiver education to promote positive health outcomes and continuity of care.
Support patient care coverage and staffing needs as a PRN Field Nurse, including weekday/weekend shifts, after-hours on-call rotation, and travel to participants' homes, long-term care, and assisted living facilities; minimum 4 hours per scheduled shift with additional hours based on census and operational needs.
