About The Position
Ohana Pacific Health is seeking a Home Health Registered Nurse to provide nursing care directly to patients in their homes. The nurse provides skilled nursing care to patients requiring substantial and specialized nursing skills and works closely with patients, families, physicians, and interdisciplinary team members using a case management model. The role promotes effective, collaborative communication among care disciplines, provides leadership and direction to an interdisciplinary team in achieving individual patient goals, and completes a comprehensive history and physical assessment when a patient is admitted to the agency. The nurse also consults with and educates patients and families about health and wellness in the home.
Requirements
- Bachelor of Science in Nursing (BSN); an Associate Degree in Nursing (ADN) may be considered instead with at least six years of direct patient care experience in home health or a related clinical setting.
- Current Registered Nurse license in the State of Hawaii.
- BLS certification.
- At least one year of experience in a hospital.
Nice To Haves
- Medicare Home Health Agency experience preferred.
Responsibilities
