About The Position
Empath Home Health, a member of Empath Health, is seeking a compassionate Weekend Baylor RN Home Health (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes throughout Sarasota & Charlotte counties (Sarasota to Port Charlotte area). Requirements
- Graduate of an approved nursing program with an active Florida RN license
- Minimum of 1 year of direct patient care experience
- Willingness to participate in weekend and holiday on-call rotations
Nice To Haves
- BSN or MSN degree
- Prior home health RN experience
- Experience using Homecare Homebase (HCHB) EMR
- Strong assessment, critical thinking, and patient education skills
Responsibilities
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled nursing services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
