About The Position

Mobile Health Team Inc. is seeking an experienced Registered Nurse - Home Health for a 13-week travel assignment in Wisconsin Rapids, WI. This role involves providing direct patient care in a home health setting, including assessments, medication administration, wound care, patient education, and care coordination. The position requires flexibility in shifts, potential for floating to other territories, and participation in an on-call rotation. Charting is to be completed in EMR timely, with specific experience in Oasis required. The nurse will assist in case managing patients in the Stevens Point/Plover area.

Requirements

  • Minimum of 2+ years of home health experience.
  • Travel experience.
  • BLS/BCLS (AHA or ARC) certification.
  • Active WI or Compact license.
  • Valid Driver's license.
  • Oasis charting experience.
  • Experience with Epic Remote Client/Remote Care.
  • Excellent problem-solving and critical thinking skills.
  • Strong organizational skills and ability to multitask.
  • Willingness to participate in continuing education and training programs.
  • Strong attention to detail and ability to work independently.
  • Strong communication, professionalism, and reliability.
  • Must pass a drug screen and background check.
  • Complete additional facility onboarding forms as assigned.
  • Must live outside 50 miles from the facility.

Responsibilities

  • Assess and chart observations of the patient’s condition at each visit.
  • Complete evaluation tasks, including reviewing medication and vital signs.
  • Administer Physician-prescribed medication.
  • Dress or redress wounds and assess the healing progress.
  • Provide education to patients and families on proper home health care procedures and strategies.
  • Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
  • Keep the patient comfortable through palliative care.
  • Recommend tools or devices that may improve the quality of life for the patient.
  • Oasis admissions, all time points, episode management, dr calls and coordination of care, documentation in EMR timely, clinical competencies in wound, IV, ports, Lab draws, chronic disease management.
  • Assist in case managing the Stevens Point/Plover area patients.
  • Triage calls during on-call shifts and determine if a phone consultation or patient visit is required.
  • Respond to answering service calls within 15 minutes.
