Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
  • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • 2 years of hospital or home health experience strongly preferred.

Responsibilities

  • Provide direct patient care, including triage, blood draws, wound care, and execution of physician orders while monitoring and documenting patient conditions.
  • Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team (IDT).
  • Coordinate care across disciplines and support discharge planning to ensure continuity of care.
  • Educate patients, caregivers, and staff while participating in team and care plan meetings.
  • Participate in on-call rotation and travel to patient residences or care facilities; part-time (20 hrs/week) with flexible schedule and documentation options.
