About The Position

Sage Group HealthCare Staffing is seeking a Home Health Registered Nurse for a 13-week travel/contract position in Bloomington, IN. We are accepting both local and traveler candidates. This role involves providing and coordinating care for assigned patients in their homes, developing and implementing nursing care plans, supporting patient recovery, and empowering patients and families through education on disease management. The nurse will also be responsible for case management, clinical oversight, and ensuring adherence to nursing practice standards and facility policies. Key duties include assessing patients, documenting visits within 24 hours, and notifying physicians of significant changes in patient status. The territory to be covered includes Bedford/Bloomington and surrounding areas.

Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Home Health Nursing experience: 1 year within the past 3 years - Required
  • OASIS experience - Required
  • Home Care Home Base (HCHB) charting experience - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • ASN or an equivalent – Required
  • RN License for State of Oklahoma or Compact - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned patients in comfort of their homes.
  • Develop and implement nursing care plans including Start of Care and admissions as assigned.
  • Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
  • Empower patients and families to reach healthcare goals by educating them on disease management
  • Case management and provide clinical oversight to interdisciplinary
  • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status
  • Document and complete charting for all patient visits within 24 hours.
