About The Position
Sage Group HealthCare Staffing is seeking a Registered Nurse Home Health for a 13-week travel/contract position in Bloomington, IN. This role involves providing and coordinating care for assigned Home Health patients, developing and implementing nursing care plans, and educating patients and families on disease management. The nurse will deliver Home Health nursing care and services within the scope of practice and facility policies, ensure physician orders are transcribed correctly, assess and document patient status, and notify physicians of significant changes. Charting is a key responsibility, and the role covers Bloomington, IN, and surrounding areas.
Requirements
- Registered Nurse: 2 years - Required
- Recent/Current Home Health Nursing Experience: 1 year within the past 3 years – Required
- OASIS experience: Recent/current - Required
- Home Care Home Base (HCHB) charting experience: Recent/current - Required
- Must be Covid Vaccinated - Declination/s accepted for this position, Medical or Religious exemptions (if qualified) may be considered on case to case basis
- ASN or an equivalent – Required
- RN License for State applying or compact if accepted in the state - Required
- BLS Certification from AHA - Required
Nice To Haves
- BSN or an equivalent – Highly preferred
Responsibilities
- Provide and coordinate care for assigned Home Health patients
- Develop and implement nursing care plans for patients as assigned.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting
