About The Position

This position involves performing home care visits, collecting ongoing data, and implementing the Plan of Care (POC) under the direction of the RN Clinical Supervisor. The role requires providing skilled nursing care in patients' homes as ordered by the attending physician, utilizing critical thinking and independent decision-making skills. Responsibilities include timely and appropriate documentation, patient education, effective communication with physicians and colleagues, and promoting continuity of care. The position offers a flexible schedule with unlimited earning potential and is part of a growing Home Health Department serving Sandusky and surrounding areas. Requirements

  • Graduate of an accredited school of professional nursing
  • Currently licensed as an RN through the Ohio State Board of Nursing.
  • BLS/CPR Certification required
  • Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy.
  • Employee must provide proof of personal automobile liability insurance coverage upon request.
  • Independent decision-making skills and abilities to render nursing care in an independent setting.
  • Ability to rotate “On Call” after hours, weekends and holidays.

Nice To Haves

  • BSN preferred
  • Prior Nursing experience in Hospice or Home Health setting preferred.
  • At least one year’s medical/surgical nursing experience preferred.
  • Experience with Electronic Medical Records (EMR) software is preferred

Responsibilities

  • Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC.
  • Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care.
  • Autonomy within your position, use your training, critical thinking and knowledge of your patients to make decisions
  • Submits clear and appropriate documentation in a timely manner and documents patient education.
  • Communicates effectively to physicians, supervisors, and co-workers in a timely manner.
  • Promotes continuity of care with appropriate admissions, transfers, and charges.
  • Other duties as assigned.
