About The Position

Sage Group HealthCare Staffing is seeking a Home Health Registered Nurse for a 13-week travel/contract position in Indianapolis, IN. This role is full-time and involves providing and coordinating patient care in their homes, developing care plans, supporting patient recovery, and educating patients and families. The nurse will also be involved in case management and clinical oversight, ensuring care is provided within professional standards and documenting all patient visits within 24 hours. The coverage area includes Indianapolis and Lafayette.

Requirements

• Registered Nurse: 2 years experience required.
• Recent/Current Home Health Nursing experience: 1 year within the last 3 years required.
• OASIS E experience: 1 year required.
• Home Care Home Base (HCHB) charting experience required.
• Must have valid Driver’s License and Personal Auto Insurance.
• Must be Covid Vaccinated or provide medical or religious exemption.
• ASN or an equivalent required.
• RN License for State of Indiana or Compact required.
• BLS Certification from AHA or American Red Cross required.

Nice To Haves

• BSN or an equivalent highly preferred.

Responsibilities

• Provide and coordinate care for assigned patients in the comfort of their homes.
• Develop and implement nursing care plans including Start of Care and admissions as assigned.
• Support patient recovery and ability to stay at home.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Case management and provide clinical oversight to interdisciplinary teams.
• Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status.
• Document and complete charting for all patient visits within 24 hours.
