About The Position

Home Health Care Solutions is seeking a Full-Time Registered Nurse to provide skilled nursing care to patients within the LaPorte area. This role involves providing skilled nursing care and case management in a home health setting, collaborating with an interdisciplinary team, performing patient and family education, and maintaining accurate clinical documentation.

Requirements

• Current RN license in the State of Indiana.
• Strong clinical assessment, communication, and organizational skills.
• Valid driver’s license and dependable transportation for travel within the LaPorte territory.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Provide skilled nursing care and case management in a home health setting.
• Collaborate with an interdisciplinary team to develop and implement individualized care plans.
• Perform patient and family education to promote independence and optimal clinical outcomes.
• Maintain accurate and timely clinical documentation.
