Health Link Home Health & Hospice is seeking a compassionate and organized Registered Nurse (RN) to join their Peninsula team in San Mateo County. This full-time position is designed for nurses who value autonomy and support. Requirements
- Current, active California RN License is REQUIRED to apply.
- Minimum 1 year of professional nursing experience required.
- Valid California Driver’s License and reliable transportation are mandatory.
- Strong clinical judgment, organization, and the ability to work independently.
Nice To Haves
- Home Health experience is preferred, but we are willing to train strong hospital/SNF nurses.
Responsibilities
- Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
- Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
- Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.
- Empower patients and families to manage their conditions safely at home.
- Maintain accurate, timely clinical notes on company-issued iPads.
